Class Absence Request Form
Calvary Chapel Bible College requires 100% attendance. All attendance issues will be handled by the Registrar’s Office, not your instructor. Class Absence Request forms must be turned in to the Registrar within two class sessions from the date of the absence (two weeks for weekly classes or two days for block classes). Students are held responsible for the information covered in the class they missed.
Class Withdrawal Form
Withdraws can only be applied for in the first 5 weeks of a course and are entered as a “W” on the student’s transcript. W’s do not affect the students overall GPA. This form must be reviewed and approved before the withdrawal is considered official. Deadline for Course Withdrawal forms are due to the Registrar no later than the end of fifth week from the “start date” of the course in order to receive a “W”. Forms submitted after the fifth week will not be accepted for consideration, resulting in an automatic “UW” (Unofficial Withdrawal) grade. UW’s affect the students GPA the same as an F grade.
Refunds for tuition costs are available until the end of the third week of the semester. Please refer to our policy located under the Tuition and Fees page.
