Ministry Applications

If you would like to serve in any capacity here at CCCV, you will need a ministry application on file.
1) Click the icon below to download the application to your computer.
2) Completely fill out the application in Adobe Reader. Click HERE to download Adobe Reader free. Make sure to save your working application periodically so you don’t lose any information.
3) Click the Submit Button at the bottom of the form. This will save a finished copy of your application to your computer and email the application. If you do not have an email program installed on your computer, you will need to open up your email service (Gmail, Hotmail, Yahoo, Etc.) in your browser (I.E., Safari, Firefox, Etc.), attach the saved application to a new email and send it to contact@calvaryccv.org.
